0:03

Hello. Now, we're going to go over

0:05

medical records and we're also going to

0:08

go over archiving. So, this is something

0:12

that you really need to know because

0:14

you're going to be given a lot of

0:16

medical records from either Trisha or

0:19

Judy. They are the current aology

0:21

aiologist at the moment. And what you

0:24

need to know is where to be able to save

0:29

and put the students information. So

0:33

first we're going to be in the aiology

0:36

Google drive which is in your shared

0:38

drives. Then you're want to go ah you're

0:41

going to want to click on one student

0:43

files.

0:45

Once you click that you can see all the

0:48

student names and their birth dates. So

0:51

this is where you're going to be able to

0:52

find your students. And this is where

0:54

you're going to save your records. So

0:58

when they give you the record, you're

1:00

going to have it in either your Google

1:02

Drive or in your one drive. So for

1:05

example, for me, I usually like to scan

1:07

the medical records in my one drive. And

1:09

so I will find it right here. And I've

1:12

already done that one. As you can see,

1:13

it's already checked off. Going to go

1:16

into downloads. And as you can see, I

1:18

put you need to put the date which the

1:23

medical record um has on there. So if at

1:29

the beginning of the information release

1:32

form, it has just like with this one, uh

1:36

January 16, 2026, that is the year that

1:40

you're going to put down. And then

1:41

you're going to put CH,

1:43

which means Children's Mercy Hospital.

1:46

If it is um some other hospital, make

1:50

sure to write that down. And then you're

1:52

going to put either information release

1:54

form, hearing aid information, colear uh

1:59

colear implant information or audio or

2:04

any other uh kind of documentation that

2:06

it is and you will specify it. But

2:10

you'll see a lot of examples to know

2:12

what to be able to save it as. So for

2:15

this student, I've saved it as that and

2:17

then I would find them in the system.

2:19

You could go through here and find the

2:21

student, but it's it's really long. So

2:24

what I would recommend is you would go

2:27

in and I'm just going to use an example

2:29

of a student and say someone with the

2:33

last name of Lee. So we're going to type

2:35

in Lee and then we're going to press

2:37

enter.

2:38

Now, there's a lot of lees in here. And

2:42

also, you're going to see that there's

2:43

other files where it just has Lee. So,

2:47

something that I usually like to do to

2:49

help me is I will go over here and click

2:51

on the button for type. And then I'm

2:54

going to click folders. So, then it's

2:56

only specifically folders of the

2:59

students. Now, like you see, we see Lee

3:03

Royal and then Lee Royal right here.

3:07

Just so you could see at the bottom this

3:10

says student one royale and this is

3:12

within the aiology

3:15

um file but as you can see over here

3:18

this is in student folders but this is

3:20

in the DHH. When you are saving medical

3:23

records you need to make sure that it's

3:25

one student files this is under aiology

3:28

because these are aiology records they

3:31

are medical. So make sure to pay

3:34

attention to that. And a good simple

3:36

rule is that if it doesn't have a birth

3:38

date, then it's in DHH. If it does have

3:40

a birthday, it's in aiology.

3:44

So, you're going to save all the

3:45

personal files within here. And then

3:47

also, you're going to make sure to save

3:50

and go down to synergy. So, click into

3:54

here. I'm going to quickly log in.

4:02

Let that load. We'll just go into this

4:04

students just as an example.

4:07

And then as you can see the student has

4:10

a lot of things but you're going to make

4:12

sure as you can see all these little

4:15

things of demographics, parents, teams,

4:18

additional um uh HOC documents. You want

4:23

to make sure that you're under

4:24

historical documents at all times. And

4:27

then when you scroll down,

4:31

you will see uh some files down here

4:35

that pretty much show where to be able

4:38

to upload your um audio. This is a place

4:42

that you want to put it because this is

4:44

for students IEPs or um 504s. So, you

4:50

would just click add

4:54

and then wherever the students audio um

4:58

that you saved it under, you're going to

4:59

add it in and then it's going to be

5:01

added into the system. So, that's how

5:03

you would do it with Synergy. And you

5:06

just need to drag and drop into a

5:09

student's file for the Google Drive.

5:12

Now, going back to student files real

5:14

quickly, I'm going to go under archive

5:17

because this is a very important thing

5:18

that you're going to have to do mostly

5:21

near the end of the year.

5:23

And I also made sure to put a good

5:26

checklist so then you know what to save

5:29

and also some instructions. So, if

5:31

you're not too sure, please look at

5:32

these for additional information. But

5:36

what you're going to do is you're going

5:37

to gather all of the files that are

5:39

going to be retired. So, for example,

5:43

this year is 2026. And so, you're going

5:46

to look for the students who were born

5:48

in 2006 because they would be over 20

5:51

years old and they would not be in

5:53

school anymore. So, they need to be

5:55

retired. And what you're going to do is

5:58

just like you can see, you're going to

6:00

put May and then the year. And you're

6:03

going to save the students file within

6:06

there. So as you can see all the records

6:09

are in here. And also I want to

6:13

emphasize because some of the students

6:15

will have very bulky folders. And this

6:18

is because they have a lot of things

6:19

that needed to be um archived for them.

6:24

But you need to focus on only Blue

6:27

Valley forms because we are just focused

6:31

on what Blue Valley has pretty much

6:35

documented. If it's from any other

6:37

medical place, you don't need to worry

6:39

about that. That's for them and they

6:42

have that record. We are saving our

6:44

records. So BV audiograms, BV hearing

6:48

screenings, OAES,

6:50

chaps, life, uh, sifter, hearing aid

6:54

waivers, amplification, aiology reports,

6:57

anything that is Blue Valley you want to

7:00

save and that is what's going to be

7:02

archived. You do not need to archive or

7:05

save anything that is from other um

7:11

medical places,

7:13

but students will already have that in

7:16

there. So, just keep that in there. It

7:19

So, as you can see, like this one's from

7:21

Children's Mercy. These are from

7:23

Children's Mercy. These just stay in

7:25

there. But your main concern when

7:28

archiving for us and when you are

7:31

retiring the file is to just save the

7:35

documents that is Blue Valley documents.
