0:01

Welcome coders to introduction to ICD10

0:06

CM. This is part one. I'm Mrs. J and I'm

0:12

so excited to begin this journey with

0:16

you.

0:17

Now, this may be your very first

0:21

experience with medical coding, and

0:23

that's a big deal. So, we're going to

0:25

start with ICD10 CM. This is the

0:29

foundation of all coding systems.

0:34

ICD10m, if you didn't know, are

0:37

diagnosis codes. Yes, this is what the

0:41

doctor says is wrong. This is what the

0:46

doctor

0:48

really this is what justifies the doctor

0:53

doing anything. All right. So, that is

0:55

going to make sense in a little bit. But

0:59

let me tell you what's going to happen

1:02

in this session. We're going to learn

1:05

how to look up codes. We're going to

1:07

understand code structure. We're going

1:10

to learn how to apply codes. And over

1:13

the fourpart series, it's going to make

1:16

sense. We're going to study we're going

1:19

to study

1:21

intense.

1:23

We're going to have an intense but basic

1:26

introduction to ICD10 CM. All right,

1:30

coders. So,

1:33

grab your coding manual

1:36

and let's get started.

1:40

Goals of the presentation. Number one,

1:43

we're going to discuss the history of

1:46

ICD10 CM, the structure of an ICD10 CM

1:50

code. We're going to demonstrate how to

1:53

look up an ICD10 CM code. We're going to

1:57

provide you with scenarios and quizzes

1:59

to test your knowledge. And we're going

2:02

to demonstrate some of AMCI ICD10CM

2:07

techniques. But first,

2:11

2:15

2:44

2:48

2:50

2:55

All right, we're going to begin with how

2:57

to look up a code in ICD10 CM in the

3:01

manual. And at AMCI, we say there are

3:04

three basic steps. Step number one, look

3:09

up the main term in the alphabetic

3:13

index. Number two, you'll need to verify

3:17

the code in the tabular list. And three,

3:22

review all guidelines. That's it. One,

3:27

two, three.

3:31

And after you look up that code, it can

3:35

be up to seven characters long. Now,

3:39

enough about the code right now. We're

3:42

going to talk about the code structure a

3:44

little later, but I want to have a

3:47

discussion about the history of ICD10.

3:54

All right. So before we talk about the

3:57

history of ICD10,

3:59

let's talk about

4:02

the history.

4:04

ICD

4:06

is the acronym for International

4:09

Classification of Diseases and it was

4:11

first developed by the World Health

4:15

Organization or WHO WH.

4:20

Now

4:22

this happened in 1948 when it took over

4:25

the responsibility

4:27

of the international classification of

4:31

diseases. So let me just give you a

4:34

little backstory. In 1893, the original

4:38

system was called the international list

4:43

of causes of death. And it was developed

4:46

by a French gentleman named Bertilian.

4:50

And he did this he he compiled this list

4:53

from about the 1900s until 1945.

4:58

And it went through many revisions. And

5:02

then in 1948

5:05

the WHO adopted that list and they

5:09

published ICD6

5:14

and this was the first version of ICD

5:19

and they

5:21

I don't want to say modified it but they

5:25

augmented it and they included both

5:30

morbidity and mortality data. So it just

5:34

wasn't a list of causes of deaths. So it

5:39

started developing

5:42

this list of diseases.

5:46

Now since then who has managed the

5:50

updates? They had an ICD9

5:53

and that was released in 1979.

5:57

They had and they also developed ICD10

6:01

which was endorsed in 1990

6:06

and ICD1

6:09

was officially adopted by the World

6:12

Health Assembly in 2019 and it became

6:16

effective in 2022.

6:20

Now you might say, well why aren't we

6:23

talking about ICD

6:26

11? Well,

6:30

that is the international

6:33

classification.

6:35

ICD

6:38

10 or any ICD in the United States has a

6:43

CM at the end. It is clinically modified

6:47

for the United States. All right. So,

6:51

let's talk about the history of ICD10

6:56

CM. This is what we use in the US. All

7:01

right. So, the World Health Organization

7:04

or WHO and they're headquartered in

7:08

Switzerland. They developed or yeah they

7:11

developed

7:13

ICD10

7:15

in the 1990s

7:18

right and ICD10

7:21

are used by com countries throughout the

7:26

world.

7:28

Then in 1994,

7:31

the United States Department

7:36

CDC, Center for Disease Control, they

7:38

have a sub um entity called the National

7:43

Center of Health Statistics. And this is

7:46

the part of the government that manages

7:50

all births and deaths.

7:52

Well, they adopted ICD10 and they felt

7:57

it needed to be clinically modified for

7:59

the United States, right? So, in 1999,

8:05

the WHO approved the US's clinical

8:09

modification

8:11

and ICD10

8:13

CM was born,

8:17

but it wasn't implemented until 2015.

8:22

when the NCS

8:26

officially released ICD10 CM.

8:31

Oh, and by the way, there are some other

8:35

key players in the clinical modification

8:39

of ICD10.

8:43

Center for Medicare and Medicaid

8:45

Services,

8:47

American Hospital Association,

8:51

and AHEA. Yes, AHEA. All four of these

8:56

entities are called the four cooperating

8:59

parties, and they help maintain and

9:02

manage ICD10

9:05

CM.

9:07

And finally, Health and Human Services

9:11

mandates that all covered entities under

9:16

HIPPA

9:18

use ICD10CM

9:21

code set. So in a nutshell, all

9:25

providers who who under HIPPA who follow

9:29

HIPPA have to use ICD10 CM. And no,

9:35

ICD101

9:38

has not yet been implemented in the

9:41

United States.

9:46

All right. So as you can see, I love

9:49

history. I am a history buff. I love it.

9:53

But I also like facts as well. And I

9:57

like fun facts, too. So, let's talk

9:59

about some facts, some basic facts about

10:03

ICD10 CM.

10:06

Fact number one, ICD10CM is estimated to

10:11

have 70,000 codes. Number two, there are

10:16

guidelines and they are divided into

10:19

four sections and appendix. So there are

10:23

guidelines throughout this manual

10:27

and

10:29

these are the type of guidelines that

10:31

they have. They have section one

10:33

guidelines. These are your primary sets

10:35

of guidelines. This is where you can

10:37

find your conventions, general and

10:40

specific coding guidelines. Then you've

10:43

got section two guidelines. This is your

10:47

principal diagnosis selection. If you

10:50

are interested in facility coding, this

10:54

is where you get your guidance. Also

10:58

section three, you get guidance for

11:00

reporting additional diagnosis. And

11:03

again, these two are for facility coding

11:07

primarily. Section four, this is the

11:10

outpatient guidelines. This is where

11:14

professional coders and outpatient

11:17

facilities find their guidance.

11:20

Also, you've got appendices. There are

11:23

eight of them. There are guidelines in

11:26

there as well. And there are 22 chapters

11:30

of specific coding guidelines.

11:36

And just so you know, there is a code

11:41

for almost any diagnosis. I mean any

11:47

there's a code for assault by a wet
